Blood HBV DNA in newborns from HBsAg-positive mothers might be predictive of immunization response

Background: In Ivory Coast, as in the rest of sub Saharan Africa, despite introduction anti-hepatitis B vaccine, toddlers and children remain a segment population frequently infected with Hepatitis Virus (HBV). The conditions early HBV transmission, notably materno-fetal route summarily documented Africa. Methods: From June to November 2018, we analyzed socio-demographical, serological, viral features related transmission expectant mothers their newborns who came for delivery at CHU Cocody Abidjan. Results: Out 1628 contacted mothers, subset 508 accepted participate study, including 163 were positive HBsAg. All 56 transmitted either HBsAg or DNA babies presenting loads >5.0 log10 IU/mL Remarkably, fates completed immune-prophylaxis drastically different 9-12 months. While majority (90%) (+)-only became anti-HBs (+), 62.5% DNA-positive failed mount proper response. Overall, 10.7% least one marker birth develop bona fide chronic infection 9 Conclusions: pregnant women above 5.0Log10 are risk maternofetal transmission. Presence blood represents better predictor anti-HBV immunization failure than presence. An adaptation procedures passive immune prophylaxis targeting specifically should be urgently considered.
